OrthoGeorgia, in conjunction with TSA Sports Medicine, will offer FREE sports physicals on Saturday, May 5th beginning at 10:30 a.m. at OrthoGeorgia.
All rising 6th – 12th grade athletes who plan to participate in sports for the 2018-2019 school year are required to have a current physical on file.  It is strongly suggested that ALL students who plan to participate in a sport, regardless of age, get a physical.

To receive your free physical you must download, print, and bring the required forms with you before you arrive.  Students will be evaluated according to the order in which they arrive. Parents are encouraged to fill out forms for students to ensure accuracy.

Parents, we are not using the physical form system of PlanetHS this year.  If you will please fill out these paper forms and turn them in to Jody Burnett at the time of the OrthoGeorgia free physical, we’ll take care of the rest.  There are 4 forms to complete, three are for parents to fill out prior to the OrthoGeorgia visit.  Parents fill out the Release for Treatment Form, Preparticipation History Form, and Parent Concussion Awareness Form.  The physician will complete the Physical Examination Form.  Upon your request, I will be able to make you a hard copy or email you a copy of your child’s completed physical form.  Hopefully, this will be a simpler solution for you as parents and for me as Eligibility Preparer!
